Assessment and Planning Visit: Complimentary

30 minute visit to establish with the practice to review available labs, discuss and create a plan of care.
Monthly Membership: $80

Covers all around access to in-person and telehealth follow-up visits. Frequency of visits is based on your needs.
Medications:​

Prescribed medications will be sent to your local pharmacy as available. Commonly, we will utilize FDA-designated 503a compounding pharmacies for certain medications which can be more cost-effective for the patient and will ship directly to the patient's home following receipt of paid invoice.
